In Person:  If you choose to complete your application in person, then you must first call the Social Security Administration at 1-800-772-1213 to schedule an appointment with your local Social Security office.  On the date of your appointment a representative at your local Social Security office will help you complete your application.  Make sure to bring all required documents with you at the time of your appointment.

By Telephone:  If you choose to complete your application by telephone, then you must call the Social Security Administration at 1-800-772-1213 to schedule an appointment for you to complete your application.  On the date of your appointment, you will receive a phone call from a representative who will assist you with completing your application.

Make sure that you set aside a period of time where you will be uninterrupted and able to focus on the application.  Also, be sure to have a copy of any required documents in front of you while completing your application.  After your appointment is completed, the representative will mail you a copy of your completed application.  You must sign the completed application and return it to the Social Security office in order for your application to be considered complete.

Online:  You can complete your application for Social Security disability benefits online by accessing the following link:  www.socialsecurity.gov/applyfordisability.  You will be given detailed, step-by-step instructions on how to complete your application. Note that if you are also applying for Supplemental Security Income (SSI), then you will not be able to complete your application online and will have to use one of the other two methods.  You can however still complete the required Adult Disability and Work History portion of your application online.
